Muttley Crew 5K – Registration

RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT In consideration of being allowed to participate in the Muttley Crew 5K on December 12, 2026, and any related activities (the "Event"), I, the undersigned, acknowledge and agree as follows: 1. Assumption of risk. I understand that running, walking, and participating in the Event, with or without a dog, involves risks including, but not limited to, falls, contact with other participants or animals, dog bites or scratches, weather conditions, traffic, uneven surfaces, and heart attack, heat illness, or other medical conditions, which may result in serious injury, illness, death, or property damage. I knowingly and voluntarily assume all such risks, known and unknown. 2. Fitness to participate. I certify that I am physically fit and have sufficiently trained for the Event, and that no medical professional has advised me against participating. 3. Release. I, for myself, my heirs, and assigns, release, waive, and discharge Muttley Crew Dog Rescue, its directors, officers, volunteers, sponsors, event partners, landowners, and the City and any public agencies involved (the "Released Parties") from any and all claims, liabilities, and causes of action arising out of or related to my participation in the Event, including claims caused by the negligence of the Released Parties, to the fullest extent permitted by law. 4. Dogs. If I bring a dog, I am solely responsible for the dog's health, behavior, and any injury or damage it causes. I agree to indemnify and hold harmless the Released Parties from any claim arising from my dog's actions. 5. Medical treatment. I authorize Event officials and medical personnel to provide or obtain emergency medical treatment for me if needed, and I accept responsibility for the cost of such treatment. 6. Rules and conduct. I agree to follow all Event rules and the instructions of Event officials and volunteers. Officials may remove me (and my dog) from the Event at any time for safety reasons. 7. Refunds. Registration fees and donations are non-refundable. If the Event is cancelled or changed due to weather or circumstances beyond the organizers' control, fees may be treated as a donation to Muttley Crew Dog Rescue. 8. Minors. If the participant is under 18, the parent or legal guardian completing this registration agrees to these terms on the minor's behalf. I have read this agreement, fully understand its terms, and understand that I am giving up substantial rights. I agree to it freely and voluntarily. Checking the box below has the same effect as my written signature.
